I have a quote for 15 usd/piece (if 5 are ordered) and 8usd/piece (if 10 are ordered).
Excluding shipping (15usd). And possible extra custom taxes.

Specs are:
Specs - 4-Layer FR4 1.6mm 1oz HASL Tin-lead Green Soldermask White Silkscreen
dimensions - 51.5x17.4mm

Without SMT mask! (didn't see te point to include that).

Everything is possible, just not in this format.
Unless you want to completely re-design the circuit...
